I usually don't ice, but I have been known to rub down with liniment after a tougher ride, or you could poultice with standing wraps overnight.

Hmm, I don't recall breaking in paddock boots being similar breaking in tall boots, however, I would probably give them a good dose of Lederbalsam to help soften them.

If you have some liquid DMSO on hand, you could mix a little bit with the ivermectin to help carry it through to whatever might be living in there; that's what I ended up doing to combat a few summer sores once.

Just curious, did you try putting the ivermectin on the spots? I don't know how easily you can obtain it down there, or if it's a precious resource, but if you can spare it, I'd give it a try. The strange lumps moving around makes me think burrowed parasite...

Was she a saddleseat show horse in another life? Possibly wore a tail set at some point? She may very well have some tendon/ligament damage if so, and after losing muscle just does not have the strength to lift anymore. Also, I've noticed Saddlebreds tend to not really lift the tail like other breed...
